No Surprises Act & Good Faith Estimate (GFE) Notice

At Thriving Life Wellness Center, we believe in complete financial transparency so you can focus entirely on your health, healing, and wellness goals. Under federal law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, prior to their appointment.

1. Your Right to a Good Faith Estimate

If you are uninsured or if you choose not to use your health insurance for your care at Thriving Life Wellness Center (self-pay patients), you have the right to receive a Good Faith Estimate (GFE) outlining the expected costs of your scheduled or requested services.

Upfront Pricing Clarity: You will receive a written Good Faith Estimate detailing the cost of your primary acupuncture, herbal medicine, Tui Na, or holistic consultation, along with any associated or follow-up services reasonably expected to be provided.
Timeline of Delivery: We will provide your GFE in writing upon scheduling a service at least 3 business days in advance, or upon your direct request.
No Hidden Surprises: The estimate will include expected itemized service codes, diagnostic expectations, and provider details (including our National Provider Identifier and Tax Identification Number).

2. Patient-Provider Dispute Resolution

If you receive a bill that is $400 or more above your scheduled Good Faith Estimate, you have the right to dispute the charges through an official federal patient-provider dispute resolution process.

Initiating a dispute does not affect your ongoing clinical care at our San Francisco practice.
